To all my Phillipsburg Patients

Since October, 1997, it has been a pleasure serving you. I have been very fortunate to have met and served many great people in this community. However, I have opportunity to expand my private practice to a level where I can provide better, more comprehensive service than I felt I could in my current location. Therefore, I will no longer be seeing patients in my Phillipsburg location after August 8th, 2009. All operations will be moved to my new location in Vernon NJ.

Your private health records will continue to be maintained in my new office to insure they reamin private. Any patients who wish to continue to be seen by me will see a seamless transition to the new office. I understand the distance to the new office from Phillipsburg will prevent or discourage many of you from making the journey. Copies of your records can be requested in the standard manner and forwarded either to yourself or to the eye care provider of your choice. As per my contract with the vision center, copies of the records will also be maintained at the Vision Center for the convenience of patients who wish to purchase their optical goods from that location.

There will be some adjustment as to which insurance plans we will continue to take and which ones we will no longer be participating with. Please call the office or email us to receive information about your specific plan.
